In order to participate in athletics at Mayville High School each student athlete needs to complete the following requirements

Upon completion of all requirements, the high school office will give your son or daughter a GOLD CARD. This card will be handed to the coach prior to or at the beginning of the 1st practice. Once coach has been given the card, all requirements are full and your child is ready for full participation. COACHES WILL NOT ALLOW YOUR CHILD TO PRACTICE UNTIL THE GOLD CARD HAS BEEN GIVEN TO THE COACH. DO NOT TURN IN PHYSICALS, FEES ETC TO ANY COACH, ONLY TO THE HIGH SCHOOL OR MIDDLE SCHOOL OFFICE.
PHYSICALS
The WIAA and Mayville School District requires a Physical to be completed every other year. Physicals taken after April 1 are good for two school years, if taken before April 1 it is good for the remainder of that school year and one more year.
If you have had a physical in the past year, a WIAA Alternate Year card must be turned in to the Athletic Office
If you are unsure of the date of your last physical, please call Kathy Buchanan at 920-387-7960 ext. 4000.
Concussion Agreement / ImPACT CONSENT FORM
Every Mayville Athlete must have an ImPact baseline test done every other year. Parents must sign a consent form and be made aware of concussion risks and information.
A registered athletic trainer will conduct training. Dates, time and location are yet to be determined. Coaches will inform athletes.
If you are due for an ImPact test you must have a signed consent form and attend one of these sessions

SPORT FEES
High School Fees - $50 FOR 1ST SPORT / $30 FOR 2ND / 3RD SPORT IS FREE
Junior High Fees: $30 FOR 1ST SPORT, $20 FOR 2ND/ no charge for any additional sports.
All Mayville School Fees for students who participate in a co-curricular activity are expected to be paid in full before participating in practice or the first scheduled event. Parochial/private school students participating in co-curricular activities as per Board policy will be expected to pay the same fees as students attending the Mayville Public Schools. Students currently approved for free lunch may petition to have their Co-curricular fees waived with the completion of a fee waiver form.
ATHLETIC CODE OF CONDUCT
Each student athlete and parent must sign our co-curricular code.
It is found on the district webpage -- athletics -- co-curricular code. You are expected to read and sign the agreement form. I or the head coach will go over the Points of Interest at your sport parent meeting.
